Pegorino asks for the last thing he will ever ask for: that Niko sit down with Dimitri Rascalov and take his money for a heroin deal. Every strand in the campaign has closed by this point and this is the only marker left on the map.
It is the branch point. Agreeing sends the story down the deal; refusing sends it down the revenge. There is no way to see both in one playthrough, and one of the two people Niko cares about dies at the end of either.
There is no combat. Pegorino makes the case for the deal, and the mission ends on Niko leaving the club.
It is not answered in the conversation. Roman and Kate both ring afterwards, and the decision is taken by driving to one of two markers that appear on the radar — the deal or the revenge. Nothing is committed until Niko arrives at one.
This is the last chance to do anything else. Friend activities, side work, remaining collectibles and any car worth storing are all easier to deal with now than during the three missions that follow, which run without a pause between them.
